  INSTRUCTIONS for downloading and emailing REQUIRED PHOTOGRAPHY WORK ORDER FORM: 
     1.  Download, open as a PDF, then fill in the form
     2.  "Save As" a PDF with a unique title and location  (ie  Your Name and Date)
     3.  Email the completed form as a PDF attachment to CHagenStudio@hotmail.com 
     4.  You will receive an email confirmation.  If not, follow up with an email or phone call Carol at 828-8405
